Environmental Health & Safety has launched the new EHS Assistant (EHSA) system. To ensure ease of adoption and understanding, it is rolling out in phases. During each phase, EH&S will contact and arrange an orientation followed by access to the system. Access to the system will not be granted until orientation is complete. Principal Investigators (PIs) must deactivate their CSUA under the following circumstances:

  • Employment termination or departure from UAB
  • Closure, expiration, or completion of a project
  • Discontinuation of controlled substance use

To properly terminate a CSUA, the following steps must be completed:

  1. Submit a deactivation request through the CSUA web application.
  2. Return all controlled substances to Environmental Health & Safety (EH&S) following proper procedures. EH&S accepts returns every Tuesday and Thursday from 9:00 AM to 12:00 PM.
  3. Return the CSUA binder with all original receipts, usage logs, and related documentation to EH&S.
  4. Fill out the CSUA Termination Document — the form is available at this link.
  5. Email a signed copy of the Termination Document to ehscs@uab.edu for official recordkeeping.

These actions are required to maintain compliance with UAB and DEA regulations.
