Request a Collection Kit
UAB Medical Genomics Laboratory collection kits are available to support specimen collection and shipment for selected testing services. Please review the eligibility, kit contents, and requirements below before submitting a request.
Who May Request a Kit
Collection kits may be requested by healthcare providers or provider offices submitting specimens for testing.
Available Collection Kits
Saliva Collection Kit
Test types: Next-generation sequencing or known variant testing
Kit includes:
- One Oragene-Dx Assisted Collection Saliva Kit (OGD-575)
- Collection instructions
- Biohazard bag
CAL Spot Collection Kit
Test types: NF1 testing on biopsied café-au-lait macules with SPRED1 reflex testing
Kit includes:
- Three Falcon tubes containing UAB Medical Genomics Laboratory-specific transport media
- Collection instructions
- Labels for documenting the collection site and patient information
- Biohazard bag
Requirements:
- Prior contact with the UAB Medical Genomics Laboratory at least one week before the scheduled biopsy
Neurofibroma Collection Kit
Test types: NF1 testing on biopsied neurofibromas
Kit includes:
- Three Falcon tubes containing UAB Medical Genomics Laboratory-specific transport media
- Collection instructions
- Labels for documenting the collection site and patient information
- Biohazard bag
Requirements:
- Prior contact with the UAB Medical Genomics Laboratory at least one week before the scheduled biopsy
Whole Blood Collection Kit
Test types: All
Kit includes:
- One 4 mL EDTA tube
- Biohazard bag
- Transport packaging
Before Requesting a Kit
A completed test requisition should be submitted with the kit request so the correct kit can be prepared and shipped.
CAL Spot and Neurofibroma Collection Kits require prior contact with the UAB Medical Genomics Laboratory at least one week before the scheduled biopsy. Whole Blood Collection Kits require laboratory approval.
