The UAB Division of CME will transition to a new activiy documentation and management system in August, 2025. Our existing activity record system will be unavailable during June and July.
To help facilitate the transition, we ask you to follow these guidelines:.

  • If you plan to renew your activity during the month of June or July, please use this interim application form.
  • We will use box folders to allow you to submit disclosures and attendance during June and July and will provide those links to you in a follow-up email.
  • If you are planning a live activity that will occur prior to 9/15/2025, or one that will seek commercial support, please use this interim application form and contact Phillip or Sonya to help guide you through the process.
  • If you are planning a live activity that will occur after 9/15/2025, please notify us by email and wait to submit your application in the system

To ensure that all physicians who have completed your activity get appropriate credit, it's important that you complete all elements of the application process and use the system to upload attendance records for each event.
